Player Certification with National Sports ID

Attention: Please do not proceed to this certification page unless you have completed your registration for the Tri-City Warriors Football or Cheer season.

Instructions

**Your Athlete MUST be certified by 7/14/2025 otherwise he/she cannot participate in the 2025 season. Because TCW Signing Day will be on Saturday, 6/28/2025, we will likely not offer on-site medical physicals as NSID certification needs to be completed prior. Please plan accordingly and make appointments with your athlete's medical professional or Doctor to complete the medical form template.**

How To Get My Child Verified - 3 step process

EBYFC will be certifying athletes using National Sports ID. Please take a few minutes to get your child certified on National Sports ID. Just click the link and follow the prompts to create an account, add your child and complete the 3-step process to get him/her verified.

Required Documents

You will need the following items for your child to get verified, you MUST have all the documents ready to upload before you begin this process. Do NOT begin this process until you have all of the following documents.

Once you sign up with your specific Team Link above, National Sports ID will ask you to complete the following information to certify your athlete:

Headshot photo of child.
Birth Certificate, Passport OR State Issued ID.
Report Card, Progress Report or some school document that shows the child’s current grade he/she is in.
Medical release from your family physician, dated 2025 (medical form template).
Ensure you e-sign the EBYFC Agreement to Participate waiver.
Pay the $12 NSID re-certification fee.

Sports clearance forms provided by a family doctor / physician.
Printed medical record indicating participant is cleared to participate in athletics and physical exam has occurred within the last 2 years, must be dated 2025.
Athletes physical or release to participate without any limitations must be signed in the current year from a doctor to participate.

Do I need to send my athlete's birth certificate or Passport?

No, please use the NSID website directly to submit all materials. We do not need a physical copy of your athlete's birth certificate or their Passport. Everything is to be completed and verified via the NSID website.

Once my athlete's physical is complete, do I need to send to TC Warriors?

No, please use the NSID website directly to submit all materials. We do not need a physical copy of your athlete's medical clearance. Although not required to use this template, we have a simple medical form template for your Physician to fill out on our Registration page. Everything is to be completed and verified in via the NSID website

I have a NSID email that says I'm verified, but Tri City Warriors says I'm still showing as not Verified. What should I do?

Please ensure you've joined the Tri-City Warriors Team in NSID for your registered level. This was a common issue which other parents experienced during T-Bird day and should be an easy fix if you are already registered.
